Service operating procedure for the Butterfly Advance Care Planning and Coordination Team (ACCT) Pre -discharge from Pilgrim Hospital, a person is identified as having deteriorating health and will benefit from having an advance care planning conversation, regarding their future care preferences and wishes, following their discharge.
