Palliative Care Referral Form for Community, Packages of Care and Funding pdf – SPA (Single Point of Access)

This gives you a view of the single form replaces the previous separate forms. It is to be used for all referrals to community nurses, community Macmillan Nurses, to St Barnabas Hospice and Marie Curie Rapid Response. Completed forms should be emailed to SystmOne and EMIS users have been sent a version that fills in automatically some information. The PCCC (Palliative Care Coordination Centre) remains the single point of access for all referrals, they can be contacted on 0300 020 0694. The form should not stop clinician to clinician discussions. There is a 24hr palliative care advice line: 0300 303 1754
