The Five Priorities for Care (Lincolnshire)

In Lincolnshire a cross organisational approach has been taken to developing the implementation of the ‘Five Priorities for Care of the Dying Person’ (LACDP,2014). Lincolnshire Community Health Services (LCHS), United Lincolnshire Hospitals (ULH), St Barnabas Lincolnshire Hospice and Marie Curie Lincolnshire have worked collaboratively and consulted with General Practioner representatives and Commissioners in the production of these Lincolnshire guidelines.

St Barnabas Hospice, Lincoln In Patient Unit (IPU) referral form

When referring a patient into the IPU a form will need to be completed and returned to St Barnabas inpatient unit. Use fax (number on the form), secure email at necmid.sbh.ipureferrals@nhs.net, or by uploading to SystmOne and tasking ‘IPU referrals’ to notify the team of the referral. If the form is uploaded to SystmOne, please ensure a share is in place so it can be accessed by the hospice.
