National NHS Continuing Healthcare Information and Advice Service (Beacon)

Beacon has been commissioned to provide an information and advice service across England. Information and advice is accessible in the form of free and comprehensive written guidance, and individuals are also able to access up to 90 minutes of free advice with a trained NHS continuing healthcare adviser.

Decision Support Tool (DST) for continuing health care – CHC

The DST is completed by a multidisciplinary team (MDT), following a comprehensive assessment of an individual’s health and social care needs and their desired outcomes. The DST asks the MDT to set out the individual’s needs in relation to 12 care domains in order to apply for continuing health care funding.

Skills for Care has produced a range of resources to support with end-of-life care training, these include case studies from healthcare providers that have successfully implemented training within their organisations, a mapping tool, and common core competencies for health and social care workers and national end-of-life care qualifications. Skills for Care, home of the National Skills Academy for Social Care, is the employer-led leadership and workforce development body for adult social care in England.
