Care To Learn is from The National Council for Palliative Care. It is designed to help staff improve end of life care. It provides an introduction to good practice, and is flexible so that it can be tailored to each individual learner, organisation or locality. It is aimed at Health Care Assistants and direct care staff across all settings including hospitals, hospices, care homes, the individual’s home or in housing schemes. There is a charge for the workbook but your organisation may support you.

20th September 2017 | Source: NHS England Sustainable Improvement Team and the Horizons Team

This Executive Summary has been developed to help all those involved in seeking to achieve transformational change in complex health and care environments.
